Sports or Camp Physicals

• This is briefer than a regular well child care check-up. It is designed for the children over 6 years old who only need check-ups every other year, but they need a more current check-up for sports or camp than the last one they had.
  
  
• Behavioral or school issues are not usually addressed here.

• NOTE: Please bring any preschool, school, sports or college forms with you to your child's well exams and we will fill them out at that time.  If you are dropping off any forms at our office, please note there is $10 processing fee for filling out any forms of these types of forms.  Please drop off and pick up all forms at our main office Suite B105.
