Well Child Check Ups and Physicals

• Call between 8:30 AM and 4:30 PM to schedule these appointments with your regular primary care physician.
  
  
• We recommend that you call and schedule these appointments as far ahead of time as possible, so that you have the best selection of dates and times. We have schedules for three months in advance in our computer.
  
  
• The best advice for scheduling well care during the first two years of your child’s life is to schedule the next visit as you leave the office at your well care visit.
  
  
• Physicals over 5 year of age will be asked if they need a vision and hearing test. This will be scheduled before your actual appointment with the doctor.
  
  
• Check ups are scheduled between 8:30 AM and 3:30 PM on Monday through Friday.
  
  
• We have a Satellite Office in the "C" building (C206) open Monday through Friday. Our physicians rotate through here, so make sure when you receive confirmation of your appointment that you know which office you are in.

• NOTE: Please bring any preschool, school, sports or college forms with you to your child's well exams and we will fill them out at that time.  If you are dropping off any forms at our office, please note there is $10 processing fee for filling out any forms of these types of forms.  Please drop off and pick up all forms at our main office Suite B105.
