Immunization Records

• Parents will be provided an immunization record card for their child at the 2 month visit on request.  
  
  You may also request your first immunization card during any check-up, but if there are a lot of immunizations that need to be entered on the card, we may need to mail it to you or ask you to pick it up in 2 business days.
  
  
• This record should be kept and brought back to each visit the child is receiving immunizations in order to be updated. This is an important record as it will be needed for your child to enter kindergarten.
  
  
• We will provide the first record free of charge and requests for duplicate immunization record cards will be done for a $10 charge.
  
  
• If you have lost your child's immunization record, call our main office number and request that we make another immunization card for your child. This will be ready to be picked up after 2 business days.
