Preschool, School, Camp or Sports

You may drop off or mail us preschool, school, camp or sports
physical forms. Please note that there is a $10 fee for filling out any forms. Please drop off and pick up in the Main Office in Suite B105.
- In order for us to complete this, they must have
had a physical exam within the last year if they are under 6 years;
or within the last 2 years if they are over 6 years old.
- Please
attach a note to let us know if you want to pick up the form or
if you want to us mail it to you or directly to the school or
camp.
- It is helpful if you attach an addressed envelope to ensure
that it is mailed to the correct place.
- Please allow 3 working
days for these to be filled out.
